This Agreed Order contains the verbiage necessary to utilize the services of RAS. Descriptions of the enumerated evidentiary items, however, are included only as suggested examples. Lawyers to an action may add, omit or edit these "template" descriptions in whatever fashion necessary to reach consent.

This Agreed Order contains the exact same verbiage as the standard Agreed Order provided above, except that it contains an additional provision creating a ten (10) day "notice" period in which a patient's or plaintiff's lawyer may file an objection on the basis of relevancy or privilege. If an objection is made, a separate court order or entity-specific authorization allowing release of the specific items must be entered or executed before RAS may complete the procurement. This extra provision provides the patient and/or plaintiff with an additional layer of due process.

This Agreed Order contains the exact same verbiage as the standard Agreed Order provided above, except that it contains an additional provision creating a seventy-two (72) hour period of time prior to any discovery deposition or trial date during which any counselor of record may procure items directly from custodians outside of the RAS service. Although the purpose of the RAS Alternative^™ is to limit excessive duplication by establishing RAS as a neutral conduit and repository - we realize that retaining the freedom to contact custodians directly during the final stages of preparation is often important. This extra provision provides defendant lawyers with this freedom.

This Authorization is HIPAA compliant and contains language approved by RAS. Descriptions of the enumerated evidentiary items, however, are included only as suggested examples and may be edited in whatever fashion necessary to reach agreement. This authorization may be used in addition to or in lieu of the Agreed Order above in certain circumstances.

Authorization to Release Mental Health Records and Psychotherapy Notes
As with Social Security records, mental health facilities will typically not release mental health information in response to a court order. They often require a mental health release authorization which explicitly references psychotherapy notes. This instrument authorizes release of mental health records to RAS and further authorizes RAS to disseminate the records to all counsel in a particular lawsuit.

Social Security - Consent for Release of Information (RAS Approved)
Since the Social Security Administration will not release patient records in response to a state court order, this Authorization is used in post-suit cases to procure Social Security records. This instrument authorizes release of social security records to RAS and further authorizes RAS to disseminate procured items to all counsel in a specific lawsuit.
